Medical Surveillance

 

If you need to schedule an appointment for medical surveillance, then please send a completed copy of the below referral form to schedule your appointment.

When scheduling an exam, please schedule at least 30 days in advanced.

 

Scheduling procedure:                               

  1. Request Process:

    1. Complete the SECNAV 5100/1 form identifying which medical surveillance programs the employee is enrolled in and the type of exam(s) needed - contact safetey@nps.edu for assistance

    2. Supervisor or Employee sends email appointment request to Occ Health at: dha.pom.presidio-ahc.mbx.occupational-health@health.mil

    3. Supervisor + Employee both need to be on all correspondence

    4. Subject (for email): “Annual medical surveillance needed” 

    5. Vaccine Records/Titers Results: must be included for positions with vaccination requirements.

  2. Forms:

    1. Medical Matrix paperwork (additional forms that may be required); emailed by OH clinic to the patient/employee.

    2. These forms need to be completed BEFORE an appointment is scheduled.

 

  1. Meeting invite (Outlook):

    1. After reviewing the forms, a meeting invite will be sent to the employee/patient and any parties listed on the 5100.

    2. Accept the meeting invite to confirm appointment.

    3. Employees need to follow any additional applicable instructions or forms that may be included in email.

After the Appointment:

  1. Provider Clearance:

    1. When the provider clears the employee/patient, they will electronically sign the 5100.

  2. Upload & Notification:

    1. The signed 5100 will be uploaded to the meeting invite.

    2. You will receive an email alert about the meeting update, even if the meeting date has already passed.

  3. Form Retrieval:

    1. After receiving the email alert, go to the updated meeting invite to retrieve the forms.

Timeframe:

  1. Allow the clinic up to 3 business days to upload the forms to the calendar following the appointment.
